How to Get a Library Card

​

Adults:

​

1. Must show driver's license

​

2. Must provide a postmarked piece of mail that can confirm the address

​

3 .Must complete and sign an application form

​

4. Must provide a local reference

​

5. Must give place of employment if employed

​

6. Must provide a contact phone number.

​

Students:

​

1. Must be enrolled in the Jal Junior High or Jal High School

​

2. Must complete and sign an application form

​

Children:

​

1. Children must be at least six (6) years of age & completed kindergarten

​

2. The parent or guardian of the child must:

    a. Have a current library card

    b. Complete and sign an application form for the child

​

***Lost cards may be replaced at a cost of $5.00.***
